Equine Health Professionals Discuss Surgical Antibiotic Use
Insights on preventing infections during orthopedic surgeries presented at the AAEP Convention 2024

At the recent American Association of Equine Practitioners (AAEP) Convention held in Orlando, Florida, John Peroni, a veterinary expert from the University of Georgia, explored the topic of surgical site infections (SSIs) in equine orthopedic surgeries. He noted that while SSIs are uncommon, many equine surgeons remain cautious about administering antibiotics prior to surgeries involving implants. Peroni highlighted that the risk of infection can arise anywhere from days to a year post-surgery, particularly when implants are involved. The potential consequences of an SSI can include life-threatening conditions such as support-limb laminitis, along with increased treatment costs.
Data from a study discussed by Peroni indicated a very low infection rate of 0.5% associated with arthroscopic surgeries performed without prophylactic antibiotics. He urged the need for a reevaluation of antibiotic usage, suggesting a risk-benefit analysis should guide decisions. Specific cases, including lengthy surgeries or when a horse has existing health issues, could warrant antibiotic administration.
For optimal effectiveness, Peroni advised that antibiotics should be given 30 to 60 minutes prior to the surgical incision, and he cautioned against prolonged use beyond 24 hours in minimally invasive procedures. Peroni introduced the concept of 'Care Bundles', which encompasses a set of best practices used in human medicine aimed at reducing SSIs through evidence-based protocols. This approach could enhance infection control in equine surgical practices. As the field continues to advance, the importance of consulting qualified veterinarians regarding any treatment decisions remains crucial.
